What is the purpose of the FAA 8710-1 form?
The information provided in the form is used by the Federal Aviation Administration to issue an airman certificate and rating or to inspect the airman’s eligibility for a certain certification or rating. The investigation will show if the airman is qualified to perform the duties related to the certificate or rating for which he is applying.

What do I do with the form after its completion?
The signed form is forwarded to the Federal Aviation Administration's Airmen Certification Branch.
